Quit Tobacco Program
Are you ready to quit smoking or vaping? Quitting smoking is not easy for most people, and it takes time. You do not have to quit smoking in one day. The Great American Smokeout is Nov. 20, a great day to start your smoke-free journey. Talking with a tobacco treatment specialist may provide the necessary support and accountability to see this challenge through. Southcentral Foundation’s Quit Tobacco Program is here to help.
Diabetes Awareness Month
November is Diabetes Awareness Month. More than one out of every 10 Americans has diabetes. Individuals with Type 2 diabetes commonly do not present any symptoms.
Join Southcentral Foundation’s Diabetes Education Program in raising awareness of diabetes. For more information on how to manage diabetes, ways to prevent or delay it, or help understanding your recent lab results, contact your primary care team.
Family Health Resources
Southcentral Foundation’s Family Health Resources assists customer-owners with their program eligibility for third-party insurance programs such as Medicaid, Medicare, Tribally-Sponsored Health Insurance Program, veterans’ benefits, and SNAP. This includes support during Medicaid renewals.
