COVID-19 vaccines are available in primary care. Please contact your primary care team if you have questions or concerns.

You can still get COVID-19 if you have received the vaccine, but receiving the vaccine can lessen your symptoms. Vaccines will not give you COVID-19 or result in a positive test, even if you have side effects after receiving the vaccine. However, you may contract COVID-19 before receiving the vaccine and not know it.

Symptoms such as an achy arm, fatigue, headache, and fever are possible and common after receiving the vaccine and indicates the vaccine is working. Due to how the vaccine must be ordered and thawed the night before administration, it is essential to keep your appointment and arrive on time if you scheduled a COVID-19 vaccine.

COVID-19 Vaccine Card/Verification of COVID-19 Vaccine

If you received your COVID-19 vaccine from Southcentral Foundation and have lost your card and/or need documentation regarding my COVID-19 vaccination for travel, see below.

  • Customer-owners

    For customer-owners who receive care at the Yagheli Shesh Qenq’a Anchorage Native Primary Care Center, contact your integrated care team at 907-729-3300.

  • SCF Employees

    For SCF employees: You may access your electronic employee health record MyCority from the SCF intranet homepage. In the filter menu on the upper left, enter Cority SSO to locate the link under All Tools. You may also contact SCF Employee Health at scfemployeehealth@scf.cc

  • Community Members

    Your COVID-19 vaccination cards are important for several reasons. They simplify the vaccination process, help people keep track of the vaccine they received, and indicate whether a second dose is needed. Showing proof of vaccination is becoming commonplace.

    Keep your vaccination card safe by storing it with important documents, like birth certificates and Social Security cards.

    If you don’t receive services at SCF but received your COVID-19 vaccination here and have lost your vaccination card, please visit the DHSS website to print out this Immunization Request Form (PDF) and mail or fax it to the Alaska Immunization VacTrAK program, which keeps track of the immunization records for Alaskans. Instructions on how to return the form are at the bottom of the form. Please include a copy of a supporting document that identifies the person requesting the immunization record.
