Aging Well Initiative

Southcentral Foundation supports the health and wellness needs of Elder customer-owners in a holistic, customer-driven, and evidence-based approach. Aging Well is a system-wide initiative that builds off the foundation of robust services and resources SCF already has to optimize care for Elder customer-owners. The initiative aims to optimize care for older adults and promote wellness for customer-owners as they age. This aim will be achieved by standardizing care delivery, improving communication across the system, and encouraging customer-owners to partner with their integrated care teams to create a care plan tailored to what matters most to them.

  • Includes customer-owner support systems

These range from dental and eye health, to weight management and diabetes, to mental health concerns, and also things like finances and life planning. Download the materials below based on topic and clinic location. Be sure to use materials specific to your clinic area as they have designated contact information for the specialty you would like more information in.
