Aging Well Initiative
Southcentral Foundation strives to support the health and wellness needs of Elder customer-owners in a holistic, customer-driven, and evidence-based approach. Aging Well is a system-wide initiative that builds on the foundation of robust services and resources offered by SCF.
The initiative aims to optimize care for older adults and promote wellness as they age by standardizing care delivery, improving communication across the system, and encouraging customers-owners to partner with their integrated care teams to create a care plan tailored to what matters most to them.

National Nutrition Month
March is National Nutrition Month. At Southcentral Foundation, we are celebrating this month with the theme Beyond the Table, equipping customer-owners with resources to make healthy choices now to maintain their health for a lifetime.
